Forms

Each year families and students must fill out some important forms.  Because of Covid-19, families can submit some of the forms online this year: our Student Agreement Form and the FERPA form.

Here is how:

1. Open the link to the form (in your language if available, English if not).
2. Save the form with your students’ initials.  For example: Maria Lopez is ML.
3. Fill out the form and “sign” where it asks. The blue boxes can be filled with text.
4. Attach the form to an email to our House Administrator, Laura Kyle at lakyle@seattleschools.org.
